You will act as a primary liaison between physicians, patients, family members, and hospice team. You’ll provide direct and meaningful patient care services to hospice patients and supportive care to the patient/ family with a functional case load. You’ll evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient. You’ll participate in company quality assessment and performance improvement programs. You’ll maintain all assigned patient clinical records. You’ll collaborate, inspire, and manage a mission-driven interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, Social Workers, Chaplains, Volunteers, and other support staff to enhance the quality of hospice care and the experience of our patients and families.
